How to Request Berwyn Death Records
Berwyn death certificates can be requested in three ways. All methods produce a certified copy with the same legal validity.

Go to 118 N. Clark Street, Room 120, Chicago, or to 16501 Kedzie Avenue in Markham. Bring a valid photo ID and $15 for the first copy. Extra copies in the same order are $4 each. Cash, check, and money order are all accepted. In-person requests are processed the same day.
By Mail
Mail a completed application, a photocopy of your ID, and payment to: Cook County Clerk's Bureau of Vital Records, 118 N. Clark Street, Room 120, Chicago, IL 60602. Make checks and money orders payable to the Cook County Clerk. Do not mail cash. Mail processing takes longer than in-person visits.
Online via VitalChek
Place your order at the Cook County VitalChek portal. Pay by credit or debit card. VitalChek adds a service fee. The Cook County Clerk fulfills the order and mails the certified copy to your address.

Who Can Access Berwyn Death Records
Illinois law limits who can get a certified death certificate. Under 410 ILCS 535/24, eligible requesters are the deceased's spouse, parent, child, grandparent, or sibling. Others with a direct and tangible interest in the record may also qualify. This includes attorneys handling probate or estate matters and people with a legitimate financial claim connected to the death.
You must prove your identity and show your connection to the deceased. If you are not eligible for a certified copy, a genealogical copy is available for records more than 20 years old. These copies cannot be used for legal or financial matters but are fine for family research.
Death Certificate Fees
Cook County charges $15 for the first certified copy and $4 for each additional copy in the same order. These fees are set by Illinois statute under 410 ILCS 535/25. Orders placed through VitalChek include an additional service fee from VitalChek.
