How to Request Mount Prospect Death Records

Online via VitalChek
Go to the Cook County VitalChek portal and submit your order online. Pay by credit or debit card. VitalChek charges a service fee. The Cook County Clerk processes the order and mails the certified copy.

Who Can Access Mount Prospect Death Records
Illinois law limits access to certified death certificates. Under 410 ILCS 535/24, certified copies are issued to the deceased's spouse, parent, child, grandparent, or sibling. Others with a direct and tangible interest in the record may also qualify, such as attorneys managing the estate or individuals with a valid court order.
You must provide proof of your identity and your connection to the deceased. If you do not meet the eligibility criteria for a certified copy, a genealogical copy is available for deaths more than 20 years old. Genealogical copies are not certified and cannot be used for legal or financial matters, but they work for family history research.
Death Certificate Fees
Cook County charges $15 for the first certified copy of a death certificate. Each additional copy in the same order is $4. Illinois statute under 410 ILCS 535/25 sets these rates. Orders placed through VitalChek include an additional service fee from VitalChek.
