How to Request Palatine Death Records

Include a completed form, a photocopy of your photo ID, and a check or money order made out to the Cook County Clerk. Do not mail cash. Mail requests take longer than in-person visits.
Online via VitalChek
Go to the Cook County VitalChek portal to place your order online. Pay by credit card. A VitalChek service fee applies in addition to the county fee. The Cook County Clerk processes and mails your certified copy.

Who Can Access Palatine Death Records
Illinois restricts access to certified death certificates. Under 410 ILCS 535/24, certified copies are available to the deceased's spouse, parent, child, grandparent, or sibling. Others with a direct and tangible interest in the record may also qualify. This includes estate attorneys, people with a financial claim tied to the decedent's estate, or individuals with a court order.
Proof of identity and your relationship to the deceased are required. If you do not meet eligibility requirements for a certified copy, a genealogical copy is available for records more than 20 years old. These copies are not certified and serve research purposes only.
Death Certificate Fees
The Cook County Clerk charges $15 for the first certified copy. Each additional copy in the same order is $4. These fees are established by Illinois law under 410 ILCS 535/25. VitalChek orders include a separate service fee from that provider.
