How to Request Oak Lawn Death Records

Payable to the Cook County Clerk. Do not mail cash. Processing time for mail requests is longer than in-person visits.
Online via VitalChek
Order through the Cook County VitalChek portal. Pay by credit or debit card. VitalChek charges a service fee on top of the county fee. The certified copy is mailed after the county reviews and processes your application.

Who Can Access Oak Lawn Death Records
Illinois law limits who can receive a certified copy of a death certificate. Under 410 ILCS 535/24, certified copies are available to the deceased's spouse, parent, child, grandparent, or sibling. Others with a direct and tangible legal or financial interest may also qualify. A court order is another valid basis for access.
You must show proof of identity and your connection to the person who died. If you do not qualify for a certified copy, a genealogical copy may be available for deaths more than 20 years old. Those copies are not certified and are for research use only, not legal or financial purposes.

Cook County charges $15 for the first certified copy and $4 for each additional copy in the same request. State statute under 410 ILCS 535/25 sets these fees. If you order through VitalChek, a separate service fee from VitalChek applies.
